La banda sonora de Doom Eternal fue compuesta por Mick Gordon. Se concibió como una continuación de la banda sonora del juego anterior, pero también como una especie de álbum de heavy metal antiguo y alienígena. Algunas canciones presentan letras en lenguas ficticias (como "kar en tuk", lengua argenta para "rasgar y desgarrar"), y se reunió un coro para la ocasión, con una convocatoria abierta a vocalistas de heavy metal. El coro se grabó en marzo de 2019. La letra adicional fue interpretada por el oktavista Eric Hollaway.
